He is a Shelty and he is 12 years old. He coughs or gags as if he is going to throw up but he swollows and he seems okay.

Take the dog to the vet.

We have a Papillion about the same age - maybe a little older. He does what sounds like the same thing. He was overweight for a period of time about 10 years ago. Which put stress on his heart.

So, our dog has an enlarged heart, with some fluid. He is doing ok on meds. --- Enalapril, a furosemide and pimobendan.

I am not a vet and I don't even play one on TV. If your dog has similar condition, the meds could help.

I bet your dog has caught Kennel cough somewhere. My dog who is only 10 months was doing the exact same thing. It juststarted and I had no clue what it was. It was like something was stuck in his throat. I took him to the vet and he said it was kennel cough. My dog still was very active and had a great appetite. The vet put him on a cough med and an anti botic. However, they say it will go away in about 2 weeks! Hope this helps

Yup..get your dog to the vet.
Heartworms can also be a sign of coughing...
I adopted a dog which had heartworms.
Once they are established, the treatment will either kill or cure them.
Luckily my dog survived the treatment and lived to be 16 years old, but he always had an enlarged heart from the heartworms.
The only reason he lived so long was that he got alot of vigorous exercise everyday his whole life.
The cough is very similar to kennel cough.

That is exactky what it is. He does have an enlarged heart. The doctor did place him on those meds.
